After many rewarding years of caring for patients, Plastic Surgery of Texas by HKB will be closing.
Dr. Ben J. Tittle will be available at the practice’s Dallas location (10743 Preston Rd) until May 29, 2026 for post-operative care as well as non-invasive care appointments. If you had a recent procedure or need follow-up care, please contact our office as soon as possible at 214-905-5075 so we may assist you with scheduling.
No patient appointments will be available after May 29, 2026 at our Dallas location. Following that date, Dr. Tittle may be reached at 219-754-6865.
Medical Records
Your medical records will remain confidential and maintained in accordance with applicable laws and regulations. Patients who wish to obtain a copy of their medical records may submit the Authorization for Release of Medical Records to:

Upon receipt of a completed authorization, records will be provided in accordance with Texas Medical Board rules regarding medical record retention and release.
